If you have NDIS funding, Calder Care has support services available to help you make the most of your plan.

Your lifestyle needs may change over time, which is why we recommend you update your plan every 12 months to ensure the services always meet your current circumstances and goals.

How do I go about getting a plan setup?
Step 1
Firstly, you will need to see if you are eligible to receive NDIS funding. You must be aged between 7- 65, live in Australia, and have a permanent or severe disability.
While NDIS is available throughout Australia, you can visit the NDIS website to view the detailed map.
Step 2
The next step is to either fill in an access request form (available on the NDIS website) or, if you need advice and assistance, contact your local Calder Care support provider, who can help guide you through the application process.
Step 3
Now it’s time to meet your plan manager and setup your plan. This plan is designed to help reach the goals you want to achieve.
Do I have control over the services and providers?
Yes, you have complete control over your whole plan. If you feel comfortable, you can manage it all yourself, or talk to our support coordinators for help choosing the right services to suit your individual needs.
You can switch from a self-managed plan to having a support coordinator at any time.
